With dates you can pick the format from the drop down and the format
dd/MM/yy will be in the list depending on your regional settings, for other
regions, you can type MM/dd/yy (no quotes - case sensitive) in the box to
give you what you want.
With the phone numbers, enter it exactly as you had it in the original post
i.e. (###) ###-####
(no quotes).
The switches don't mask the input, they format the output when you tab out
of the field. In the case of the date you can enter the date in any standard
date format and it will be converted to the layout shown.
Don't forget to set the field type to number or date as appropriate.
