Hi, Is it possible to make a form by Word?

Let me say, I need creat a form which will need users fill some information
such as
Name _____

Address _______

The user can put information on the line but should not be able to change
the form.

Can Word do this?

What you are talking about is what Word calls an "online form." Check this
