I got a document that has comments/balloons on the right margin of the page
in red. Each balloon has a title, for example "Deleted or Formatted," then
has text describing the change, deletion etc. to the right.

When I click the Comment Icon on the reviewing toolbar (or insert comment),
I get a colored balloon that has the title, "Comment." How can I change the
"Comment" title in the balloon to Deleted, or Formatted, etc?

A comment is a comment. To get a balloon that says "Deleted" or "Formatted,"
you must delete or format text with Track Changes turned on. For more, see
