Help...

It's bizarre - when I first go into File
Manager/Windows Explorer the icons are normal. Within a
few clicks I have Photoshop Icons on Word documents,
QuickBooks icons on PowerPoint, etc. Anybody got a clue?

Common problem. Increase the size of your icon cache. Either...
1) use a utility such as Activicons to increase the icon cache size to
at least 2048 - http://www.cursorarts.com/ca_ai.html (the option is on
its "Desktop Controls" screen) - or

2) run Regedit. Go down the tree to this key:
HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Windows\CurrentVersion\explorer
Here, you will then need to right-click and create a new "string
value." Give it the name "Max Cached Icons" and then right-click to
edit the properties and make the "value data" either 2048 or 4096.
