Is there a way for me to add a video that would play at or immediately
following Windows Vista startup? Video is an mpeg-2 file format.

Drop the shortcut to the file on the start menu at
C:\Users\<YourID>\AppData\Roaming\Microsoft\Windows\Start Menu\Programs

I created a shortcut to the video, copied into the folder you specified, but
when booting the system no video plays. What am I doing wrong?

Possibly you'd need to write a startup script that 1) calls your player of
choice; 2) opens the file in the player. I can't help you write the script
but I'm sure someone else can.
