SBS 2k3 comes with 5 cals out of the box. If I add 5 CALs, do I need to
also add 5 Exchange Server CALs?

just to expand a little on Karl's reply.

W2K3 CALs are not SBS CALs. SBS CALs include the server access (basically, a
W2K3 CAL) and also CALs for SBS components.

So hopefully, you have an additional 5 SBS CALs you wish to add, because you
can, not an additional W2K3 CALs which are of no value to you.
