Using SBS 2003, occasional incoming mail (which had been sent during the
normal business day) is held until midnight then delivered to the SBS user
at that time. More often than not, it is a short email rather than a large
one. What should I do to troubleshoot?

When you say 'held', do you mean in Exchange or somewhere outside your
network?

Do you collect mail using POP3 or receive it by SMTP?

Do the message Received: headers show where it was held up (Options for
the message in Outlook)?

What does the Message Tracking Centre (Exchange, Tools) have to say?
