I received an email with a business card signature block which contained
icons with pop up text for phone, fax ,email information, etc. I
inadvertently deleted the email and am trying to reproduce. Any help would
be greatly appreciated.

I'm not sure what you mean by "pop-ups" and can't visualize it unless I see
it.
You can use a Standard Outlook Business card as part of your signature and
that can be customized to some extent.
Go to Tools | Options | Mail format tab then Signatures button.
Create a signature and choose Business Card to insert your Business Card.
