In Outlook 2007, when I run a spell check on an email message, the
"Add to Dictionary" button is grayed out. Same thing in Word. Has
anyone else had this issue? I do have a custom dictionary defined
(custom.dic). Thanks,

Where is the custom.dic? Does outlook have write permission to it?

Go to the spelling dialog - select your Custom.dic, then click Modify. If
you cant modify it, then it means it has been corrupted. you can open and
browse it, looking for corrupt entries, or rename it and make a new one.
