I am having an issue where once in a while an email message (that I have
moved to an Outlook Folder) gets placed back into my Inbox. Has anyone else
had this happen or know of a fix for this?

Outlook 2003. Regular Inbox with Folders underneath it.

That's not an account type. Your account is Exchange, POP, IMAP, or HTTP.

While I've seen multiple reports about this with Exchange mailboxes and
while I can think of several things that might cause it, I don't have a
definitive answer. It's because of something happening on the Exchange
server, though, so were I you, I'd be speaking to the Exchange admins.

either to pinpoint what happened.
