Outlook will accept appointments with reminders, but will not remind and has

Are the appointments in the default calendar? What version of Outlook?
What type of account? Have you tried starting Outlook once with the
/cleanreminders command switch?

You didn't answer all of my questions. Is the appointment in your default
calendar? What type of account? I believe the reminder window will always
be blank unless there's an active reminder that has fired but that you
haven't yet dismissed. The command switch I described will regenerate all
reminders for upcoming appointments. Click Start>Run and in the Open field
enter

outlook.exe /cleanreminders

Be sure to include the space.
