I need to print out a full month calendar that looks like a calendar (e.g.,
not a list) and includes ALL my appointments and as well as their subject
plus beginning AND end times for each appointment. Is there a way to do this?

probably not within outlook... unless you have very few appointments per
day.

http://www.slipstick.com/addins/calendar_print.asp - try one of the word
templates and if that doesn't meet your needs, try one of the commercial
utilities.
