Are you using Forward or Actions, Forward as an iCalendar? if you use
forward (and forward as an outlook item) it will be added as tentative, just
like any meeting request. If you use ical, it's only saved if you click save
and close.
Try creating the item, save and close, then insert it into a message using
the paperclip icon.
