I keep getting a reminder every time I open MS Office that will not let me
dismiss it, Each time it reminds me and uses the current date. When I try
to delete the reminder it states:

The custom form cannot be oppened. Outlook will use an outlook form
instead. Cannot locate recurrence information for this appointment.

What can I do to eliminate this reminder?

have you opened outlook using the cleanreminders switch?
